Strahlenspätwirkungen bei Thorotrast-Ablagerungen

Zusammenfassung

Thorotrast, eine 22%ige Suspension von Thoriumdioxyd, wurde 1928/29 als Röntgenkontrastmittel zur Darstellung von Gefäßen und Hohlräumen und wegen seiner Speicherung im RES zur Hepatolienographie eingeführt. Die heute beobachteten Spätschäden werden verursacht durch chronischen Fremdkörperreiz und interne Langzeitbestrahlung. Paravasate und intrakavitäre Ablagerungen führen zu schwieligen Thorotrastgranulomen, in Leber und Milz zur Cirrhose. Nur selten sind solche Thorotrastdepots radikal zu entfernen. 260 Thorotrasttumoren sind inzwischen publiziert worden. Es handelt sich meist um inoperable Leber- und Gallengangcarcinome. Nierengeschwülste nach retrograder Pyelographie oder Malignome nach intrakavitärer Applikation an anderen Organen können gelegentlich noch radikal entfernt werden. Die Milzexstirpation ist nur noch ausnahmsweise bei erheblichen subjektiven Beschwerden durch die Thorotrastdepots indiziert. Die Zahl der Thorotrastkranken in Deutschland wird noch auf einige tausend geschätzt; die Mithilfe aller Ärzte bei der zentralen Erfassung und Untersuchung (Heidelberg, Homburg/Saar) wird dringend erbeten.

Thorotrast, a 22% suspension of thorium dioxide, was introduced in 1928/29 as X-ray contrast medium for visualizing vessels and cavities and for hepatolienography because it was deposited in the RES. The late effects observed to-day are caused by chronic foreign-body irritation and prolonged internal irradiation. Extravasations and intracavitary deposits lead to indurated Thorotrast granulomas and to cirrhosis of liver and spleen. Only rarely is it possible to remove such Thorotrast deposits radically. 260 cases of Thorotrast tumors have been published. They were mostly inoperable carcinomas of the liver and biliary passages. Kidney tumours following retrograde pyelography or malignant tumours in other organs after intracavitary application can occasionally be removed radically. Removal of the spleen is indicated only exceptionally in the presence of considerable subjective symptoms due to Thorotrast deposits. The number of Thorotrast sufferers in Germany now is estimated at several thousand. The collaboration of all doctors in collecting cases centrally for examination (Heidelberg, Homburg/Saar) is urgently requested.
