Solitäre Ektasie von Halsvenen

Summary

We report on 7 separate ectasias of the neck-veins; 4 of the v. jugularis externa, 2 of the v. jugularis interna and 1 of the v. jugularis anterior. For recognition purposes, swelling of the neck on pressure (Valsalva’s test) and its subsidence on steady breathing are characteristic. The diagnosis can be confirmed by the introduction of a contrast medium, in which direct injection is the most common method employed. Completely thrombosed formations are easily confused with a solid tumor. In strong or rapidly growing ectasias, an operation—usually exstirpation—is indicated. By reducing the dilated portion to the normal vascular size, we were able to remove one fusiform ectasia of the v. jugularis interna.

Zusammenfassung

Es wird über 7 solitäre Halsvenenektasien — 4 der V. jugularis externa, 2 der V. jugularis interna und 1 der V. jugularis anterior — berichtet. Für die Erkennung ist das Auftreten einer Schwellung am Halse beim Pressen (Valsalvascher Versuch) und ihr Wiederverschwinden bei ruhiger Atmung charakteristisch. Die Diagnose läßt sich durch Kontrastmittelfüllung sicherstellen, wobei zumeist die direkte Injektion angebracht ist. Völlig thrombosierte Gebilde werden leicht als solider Tumor verkannt. Bei stärkeren oder rasch größer werdenden Ektasien ist die Operation — meist Exstirpation — angezeigt. Eine spindelförmige Ektasie der V. jugularis interna haben wir dadurch beseitigt, daß der erweiterte Abschnitt auf die normale Gefäßstärke reduziert wurde.
