Summary
During operative therapy of pseudo-arthrosis, the fragments should be stabilised, where possible. However, bone-transplantation still possesses quite a wide range of indications as a mode of treatment, both combined with osteosynthesis and in the absence of this. The transplant material with the greatest changes of success is autologous bone. Autologous bone-transplantation becomes absolutely necessary in defective pseudo-arthrosis, but may also be employed in pseudoarthrosis of the bone-ends or the fine bones, when sufficient stabilisation is not possible. Bone-transplantation is also indicated as a second process, where healing does not take place in spite of osteosynthesis. Spongiosa grafts are particularly significant in infected pseudo-arthrosis, since autologous spongiosa is surprisingly insensitive to infection. Grafting can be undertaken straight in the cleared focus. This is also very suitable for forming an osseous bridge or block outside the infected area, as for example in tibio-fibular block-formation in infectious tibial pseudoarthrosis. In conclusion, the opinion is expressed, that bone-transplantation as a means of treating pseudo-arthrosis, particularly in the form of cumulative chips, is technically easier to perform and carries with it less risk than stable osteosynthesis. It is therefore, even today, in many cases a completely up-to-date mode of treatment.
Zusammenfassung
Bei der operativen Behandlung von Pseudarthrosen soll die Stabilisierung der Fragmente durchgeführt werden, wo sie möglich ist. Daneben hat aber auch heute die Knochenüberpflanzung als Behandlungsmethode einen weiten Indikationsbereich, und zwar in Kombination mit der Osteosynthese und auch ohne sie. Als Transplantationsmaterial besitzt der autologe Knochen die größte Erfolgssicherheit. Die autologe Knochentransplantation wird zwingend notwendig bei der Defektpseudarthrose, aber auch bei Pseudarthrosen an den Knochenenden oder kleinen Knochen, wenn eine ausreichende Stabilisierung nicht möglich ist. Die Knochentransplantation ist auch angezeigt als Zweiteingriff bei Ausbleiben der Heilung trotz Osteosynthese. Besondere Bedeutung besitzt die Spongiosaplastik bei den infizierten Pseudarthrosen, denn die autologe Spongiosa ist erstaunlich wenig infektionsempfindlich. Die Plastik kann direkt in den ausgeräumten Herd vorgenommen werden. Sie ist aber auch sehr geeignet zum Aufbau einer Knochenbrücke oder eines Knochenblocks außerhalb des infizierten Gebiets, wie etwa bei der tibiofibulären Blockbildung bei infizierter Tibiapseudarthrose. Abschließend wird betont, daß die Knochentransplantation als Behandlungsmethode der Pseudarthrose insbesondere in der Form des Anlagerungsspans technisch einfacher durchzuführen ist und ein geringeres Risiko hat als die stabile Osteosynthese. Sie ist deshalb in vielen Fällen auch heute noch eine durchaus zeitgemäße Behandlungsmethode.
