Indikationen der Gastrostomie und Enterostomie beim Kinde

During the last few years gastric and intestinal fistulas have been increasingly used with good success during surgical procedures on the gastro-intestinal system, particularly in newborns and infants. On the other hand, as an additional procedure, they constitute a certain source of danger with additional potential complications, most of all in prematures. — Gastrostomy serves for the protection of the surgical field proximal or distal to the stomach, it also serves for decompression and improved evacuation of the gastro-intestinal tract in patients with deus, as a feeding fistula, and, finally, for dilating the esophagus with bougies. Basically it may be of value for all malformations of the gastro-intestinal tract. The indications for enterostomy are more limited. Whereas the lateral fistula of the intestinal wall with the aid of probes serves for decompression and splinting in patients with distal intestinal occlusions and meconium ileus, terminal ileostomies should only be used for a short time in emergency situations.

Zusammenfassung

In den letzten Jahren sind die Magen-und Darmfisteln bei operativen Eingriffen am Verdauungskanal vor allem des Neugeborenen und Säuglings in steigendem Maße mit gutem Erfolg angewandt worden. Andererseits sind sie als ein weiterer Eingriff, vor allem beim Frühgeborenen, eine gewisse Gefahrenquelle mit zusätzlichen Komplikationsmöglichkeiten. — Die Gastrostomie dient dem Schutz eines Operationsgebietes oral oder aboral des Magens, der Entlastung und besseren Entleerung des Magen-Darm-Kanals bei Ileussituationen, als Ernährungsfistel und schließlich zur Bougierung des Oesophagus. Sie kann im Grunde bei allen Fehlbildungen des Verdauungstraktes von. Nutzen sein. Das Indikationsgebiet der Enterostomie ist begrenzter. Während die seitliche wandständige Darmfistel mit Hilfe von Sonden bei hohen und tiefen Darmverschlüssen und beim Meconiumileus der Dekompression und Schienung dient, sollte das endständige Ileostoma nur in Notsituationen kurzfristig angelegt werden.
