Beobachtungen bei Rejektion von Nierentransplantaten

Summary

From a group of 21 kidney transplants, 3 cases are demonstrated with acute rejection-crises as reflected in the clinical course and anatomical-pathological findings, during which sensitisation is discussed. In the first case, there was a spontaneous rupture of a retransplant. In the second case, acute reversible haematuria and anuria is attributed to a second-set-phenomenon after streptococcal and staphylococcal antigen-sensitisation. In the third case, a picture of hyperacute rejection is described. The significance of humoral antibodies in early rejection is emphasized and the influence on these of heparinisation is discussed.

Zusammenfassung

Aus dem Kollektiv von 21 Nierentransplantationen werden 3 Fälle mit akuten Abstoßungskrisen anhand des klinischen Verlaufes und pathologisch-anatomischer Befunde demonstriert, bei denen eine Sensibilisierung diskutiert wird. lnm ersten Fall kam es zur Spontanruptur eines Zweittransplantates. Im zweiten Fall wird ein second-set-Phänomen nach Streptokokken-und Staphylokokken-Antigensensibilisierung als verantwortlich für eine akute reversible Hämaturie und Anurie angesehen. Im dritten Fall wird das Bild einer hyperakuten Abstoßung beschrieben. Die Bedeutung humoraler Antikörper bei der Frühabstoßung wird hervorgehoben und ihre therapeutische Beeinflussung durch Heparinisierung diskutiert.
