Zusammenfassung

Schon Quincke hatte erwogen, ob man die Cisterna cerebello-medullaris punktieren könnte; er lehnte eine solche Punktion wegen ihrer scheinbaren Gefährlichkeit ab. Und vorerst wurde kein Versuch unternommen, solche Überlegungen auf ihre Berechtigung hin nachzuprüfen. Dagegen wurde von chirurgischer Seite die Gegend der Cisterna cerebello-medullaris wiederholt operativ in Angriff genommen. Und zwar auf verschiedenen Wegen. Murphy (1907), Payr (1911) und Lossen (1914) bahnten sich zwecks Druckentlastung des Gehirns den Weg zur hinteren Schädelgrube durch die Hinterhauptsschuppe. Payr hat dann später (1916) auf gleiche Weise die Eröffnung der Cisterna cerebello-medullaris bei Meningitis serosa acuta traumatica mit gutem Erfolg durchgeführt und die große Bedeutung dieser Operation für die chronischen Formen der umschriebenen serösen Meningitis hervorgehoben. 1905 war aber schon auf Vorschlag von Westenhöfer gelegentlich einer Genickstarreepideme in 4 Fällen die Incision des Ligamentum atlanto-occipitale ausgeführt worden. Westenhöfer berichtete auf der 78. Naturforscherversammlung (1906) über die gesammelten Erfahrungen und empfahl zum Zweck der Druckentlastung im akuten Stadium wie zur Verhütung und Behandlung eines konsekutiven Hydrocephalus die Incision and Drainage des Ligamentum atlanto-occipitale.
