Adrenale noduläre Hyperplasie: Ursache eines Cushing-Rezidivs nach erfolgreicher hypophysärer Mikroadenektomie

Summary

Long-lasting stimulation by adrenocorticotropic hormone (ACTH) due to pituitary adenoma may cause nodular enlargement in the adrenal cortex. This nodule may function autonomously and cause ACTH-independent Cushing’s syndrome. We present two cases of recurrent Cushing’s syndrome following successful pituitary microadenectomy (with positive histopathology and postoperative decrease of ACTH). Both were treated with bilateral adrenalectomy and histopathology revealed macronodular adrenal hyperplasia. Cushing’s syndrome was cured after bilateral adrenalectomy in both patients.

Zusammenfassung

Nach einer langzeitigen ACTH-Stimulation (per Hypophysenadenom) kann der adrenale Kortex sich knotenförmig vergrößern. Diese Knoten können manchmal ihre Autonomität gewinnen. Autonome Knoten können die Ursache eines ACTH unabhängiger Cushing-Syndroms sein.

Wir berichten über zwei Patienten mit Cushing-Syndrom, die zuerst wegen hypophysäre Mikroadenome operiert wurden. Obwohl die Eingriffe erfolgreich waren (ACTH Senkung und Bestätigung mit Histopathologie) kamen die Patienten nach einer Weile mit Rezidiv zurück. Bilaterale Adrenalectomie hat die Patienten geheilt. Die Histopathologie zeigte, daß beide Patienten makronoduläre adrenale Hyperplasie hatten.
