Summary
To date thoracoscopy is firmly established as a treatment for spontaneous pneumothorax and primarily as a treatment for recurrences. From January 1 1990 to December 31 1994, 66 patients with 76 occurrences of spontaneous pneumothorax were treated at our clinic. In 35 cases we treated the patients with drainage therapy. Seven patients received primary thoracoscopic treatment. In 24 patients a thoracoscopic bulla resection was performed after they were treated with drainage. Ten patients underwent a thoracotomy after failure of the drainage treatment or following a persistent air leak. With a mean observation time of 37.6 months we had a reccurrence rate of 34.3% after drainage, compared to a recurrence rate of 6.45% after a thoracoscopic bulla resection. The rate of persistent air leakage also revealed a clear difference: drainage, 33.9% versus thoracoscopy, 8.45%. Because of these results we pose the question: Should a thoracoscopic bulla resection not be used as a first episode therapy?
Zusammenfassung
Die Thorakoskopie hat inzwischen einen festen Platz in der Behandlung des Spontanpneumothorax eingenommen, jedoch erst bei der Behandlung des Rezidivs. Zwischen dem 1.1.1990 und 3.12.1994 wurden an unserer Klinik 66 Patienten mit 76 Ereignissen eines Spontanpneumothorax behandelt. In 35 Fällen führten wir eine reine Drainagebehandlung durch. Sieben Patienten wurden primär thorakoskopiert. Bei 24 Patienten erfolgte nach einer Drainagebehandlung die thorakoskopische Bullaresektion. Bei zehn Patienten führten wir nach fehlgeschlagener Drainagebehandlung oder persistierendem Luftleck eine Thorakotomie durch. Bei einer mittleren Nachbeobachtungszeit von 37,6 Monaten zeigte sich für die Drainagebehandlung eine Rezidivrate von 34,3% im Vergleich zu 6,45% bei der thorakoskopischen Bullaabtragung. Auch bei der Rate an persistierenden Luftlecks zeigte sich ein deutlicher Unterschied: Drainagebehandlung -33,9% versus Thorakoskopie - 6,45%. Auf Grund dieser Ergebnisse stellt sich die Frage, ob die thorakoskopische Bullaabtragung nicht auch als Primärtherapie angezeigt ist.
