Summary
Despite certain improvements due to early diagnosis and the use of endoscopic tools, over 20% of large bowel malignancies still present themselves as an acute abdomen, most often with signs of bowel obstruction followed by perforation. A total of 949 patients have been operated for large bowel malignancies since 1984, of which 187 patients (19.7%) presented signs of bowel obstruction, including 35 (3.7%) that developed acute abdomen due to colonic perforation. Patients older than 60 were the most commonly affected of the whole group. The sigmoid colon was involved in 14 (40%) of the patients, the rectum in eight (22.8%) and the coecum in five (14.3%) of the patients. Early postoperative mortality in patients that developed perforation was 28.5%. 5-year-survival rate was 12.5%.
Zusammenfassung
Trotz erheblicher Verbesserung bei der Früherkennung von Dickdarmtumoren und auch der Anwendung der endoskopischen Untersuchungsverfahren kommt es dennoch gerade beim Koloncarcinom in über 20% zur Ausbildung eines akuten Abdomens, sowohl unter den Zeichen einer Ileussymptomatik, wie auch zur Peritonitis bei der Perforation. In unserer Klinik wurden seit 1984 949 Patienten wegen eines Koloncarcinoms operiert. Notfallmäßig mußten 187 (19,7%) Patienten wegen eines Ileus und 35 (3,7%) Patienten wegen eines perforierten Koloncarcinoms laparotomiert werden. Bei der Altersverteilung liegt der Häufigkeitsgipfel im 6. und 7. Lebensjahrzehnt. Der Schwerpunkt der Tumorlokalisation liegt mit 40% (14 pt.) im Sigma, gefolgt von 22,8% (8 pt.) im Rektum und 14,3% (5 pt.) im Coecum. Die postoperative Sterblichkeit bei den Patienten, die im klinischen Bild der Dickdarmperforation operiert werden mußten, betrug 28,5%. Die exakte Überlebensrate nach 5 Jahren beträgt in unserem Patientengut 12,5%.
