Operative Technik und Nachsorge der laparoskopischen Gastrostomie (Kader-Fistel)

Summary

Laparoscopic gastrostomy is a minimally invasive alternative to conventional gastrostomy. The stomach is fixed by a three-point gastropexy to the anterior abdominal wall and Foley catheter is inserted into the stomach using a Seldinger technique and a peel-away sheath. Enteral nutrition starts on the first postoperative day. Laparascopic gastrostomy is indicated in palliative situations and allows enteral nutrition with minimal operative trauma and short hospitalisation, even in cases with total esophageal stenosis. This is of special relevance for patients in a palliative situation.

Zusammenfassung

Alternativ zur PEG bietet sich neuerdings die laparoskopisch-ope-rative Magenfistelanlage im Sinne eines minimal invasiven Vorgehens an. Der Magen wird durch eine 3-Punkt-Gastropexie an der Bauchwand befestigt und mittels einer Seldinger-Technik ein Foley-Katheter durch eine Plastikschleuse in den Magen eingebracht und geblockt. Die Nahrungsaufnahme erfolgt am ersten postoperativen Tag nach radiologischer Dichtigkeitskontrolle. Die in hochpalliativen Situationen indizierte Kaderfistel kann auf minimal invasivem Wege dem Patienten die enterale Ernährung bei kleinstem Operationstrauma und kürzester Hospitalisation ermöglichen. Die Ernährung ist mit passierter Kost ohne spezielle Sondennahrung möglich. Dies sind wesentliche Gesichtspunkte für Patienten in palliativer Situation.
