Die Technik der konventionellen Appendektomie (Kontroverse zur laparoskopischen Technik)

Summary

The performance of an appendectomy using the conventional technique is not limited by previous abdominal surgery, expected perforation or concomitant medical disease. Additional surgery in the case of complications or intraoperative change of diagnosis is easily feasible. The way of access for surgery and the procedure itself are clinically tested, low in complications and require less technical effort (no access via trocars and their risks, no staplers except in rare cases, less dangerous electrocoagulation). There is only little difference in economical terms between laparoscopic and open appendectomy. It is the most common emergency operation in visceral surgery and every surgeon must be able to perform it at any time and anywhere. That is how the preference in teaching should be. Comfort and small scars are less important.

Zusammenfassung

Die konventionelle Appendektomie (AE) unterliegt in ihrer Technik keinerlei Einschränkungen (Vor-OP, evtl. Perforation, cardiopulm. Begleiterkrankungen), eine Ausweitung der OP ist bei Zwischenfällen oder Diagnoseänderung jederzeit möglich. Zugänge und OP sind erprobt, komplikationsarm und mit geringem technischen Aufwand verbunden (keine Trokarzugänge mit entsprechenden Risiken, Stapler nur in Ausnahmefällen, Stromanwendung weniger riskant). Die Gesamtkosten unterscheiden sich nur unwesentlich (betriebs- und volkswirtschaftlich) vom endoskopischen Eingriff. Die AE ist die häufigste viszeralchirurgische Notfall-OP, entsprechend muß jeder Chirurg sie überall und immer können. So muß auch die Präferenz in der Ausbildung sein. Komfort und Kosmetik sind erst von sekundärer Bedeutung.
