Endoskopische Behandlung des Karpaltunnelsyndroms (1-Portal-Technik)

Summary

Endoscopic release of the transverse carpal ligament (TCL) from “outside to inside” is performed by a technique familiar to every surgeon. The very short skin incision is located in a zone of the palm not subject to strain over the distal margin of the TCL. Under the palmar aponeurosis an artificial space is formed using Hegar’s dilators and then a grooved hook with a guide for the telescope is inserted into this space. The subsequent procedure is identical to that used in open incision. Under endoscopic vision, the carpal ligament is severed step-by-step using a SMILIE knife until the undamaged synovial bursa of the flexor tendons becomes visible.

Zusammenfassung

Die endoskopische Karpalbandspaltung von „außen nach innen“ erfolgt auf die jedem Chirurgen geläufige Weise. Die kleine Hautinzision liegt in einer nicht belasteten Zone zentriert über dem distalen Rand des Karpalbandes. Unter der Palmaraponeurose wird mit Hegarstiften ein künstlicher Hohlraum geschaffen, in den ein Rinnenhaken mit Optikführung eingebracht wird. Das weitere Vorgehen entspricht nun ganz dem der offenen Spaltung. Unter endoskopischer Sicht wird das Karpalband schrittweise von palmar nach dorsal mit einem kleinen SMILIE-Messer durchtrennt, bis der mattspiegelnde unverletzte Sehnenscheidensack sichtbar ist.
