Summary
The complex 3D-problem of distal aiming is reduced to three easier 2D-steps: (1) Preadjustment of an aiming handle: This defines rotation and distance of distal locking holes relative to the proximal nail end. During insertion, the nail usually bends. This is why a readjustment is necessary. (2) Re-adjustment for transverse holes: At the level of the distal nail end, a roughly placed “contact hole” is drilled. An asymmetric spacer is inserted, placed directly onto the nail and connected with the aiming arm. By this re-adjustment, direction and plane of both distal transverse holes are defined. (3) Re-adjustment for ap hole: The upper transverse hole is over-reamed (diameter 6.0 mm). A centering unit inserted into this channel locks the nail to the aiming arm. After this readjustment in the frontal plane, the ap hole is defined. Advantages: Simple and reliable; drill holes more precise; radiation-free distal targetting.

Entwicklung eines mechanischen Zielverfahrens für die distale Verriegelung, wobei das dreidimensionale Ortungsproblem in 3 zweidimensionale Lösungsschritte aufgeteilt wird: 1) Vorjustieren eines Zielbügels, damit Definition von Rotation und Abstand. Durch Insertion kommt es zur Nageldeformation, deshalb Nachjustierung erforderlich. 2) Bohren einer grobplazierten „Kontaktöffnung“(0 6 mm) am unteren Nagelende, hier wird ein spezieller Abstandsgeber durch die Tibiavorderwand auf den Nagel aufgesetzt. Damit sind Richtung und Ebene der Querlöcher definiert. 3) Nach Herstellen der Querbohrungen Überbohren des oberen seitlichen Loches auf 6 mm und Ankoppeln einer Zentriereinheit. Damit ist das ap Loch definiert, das gebohrt und mit Schrauben besetzt werden kann. Vorteile: einfach, sicher, präzisere Bohrungen, weniger Bolzendeformation, keine Röntgendurchleuchtung.
