2-Stufen-Konzept zur Behandlung der oberen Dünndarmfistel

Summary

Fistulas of the proximal gastrointestinal tract are associated with a high mortality rate because of the risk of peritonitis, sepsis and loss of protein and electrolytes. Six patients with enterocutaneous fistulas were treated with an operative two-step concept. First step: Treatment of peritonitis; acurate drainage of the fistulas and open wound treatment. Second step: Treatment of the fistula; resection of the afflicted bowel. End-to-end or end-to-side anastomosis with the jejunum and omental pedicle graft. With this two-step treatment fistulas in three patients closed primarily, the fistula output of one patient was reduced so that it closed spontaneously, while one patient died as a result of sepsis and one patient had a persistent fistula.

Zusammenfassung

Fisteln des oberen Gastrointestinaltraktes sind wegen der Gefahr von Peritonitis, Sepsis, Eiweiß- u. Elektrolytverlust mit einer hohen Mortalität behaftet. 6 Patienten mit Fisteln des oberen Gastrointestinaltraktes sind mit einem 2-Stufen-Konzept behandelt worden. 1. Schritt: Sanierung der Peritonitis. Gezielte Drainage der Fistel u. offene Wundbehandlung. 2. Schritt: Sanierung der Fistel. Resektion des fisteltragenden Darmabschnittes und Rekonstruktion End-End oder End-Seit mit Jejunalschlinge und Netzplombe. Mit diesem 2-Stufen-Konzept gelang es bei 3 Patienten, die Fistel primär zu verschließen, bei einem Pat. die Sekretionsmenge so zu vermindern, daß ein spontaner Fistelverschluß erfolgte. 1 Patient verstarb unter prolongierter Sepsis vor der Fistelsanierung. Ein Patient leidet noch unter einem Fistelrezidiv.
