Die Grenzen der Chirurgie bei Patienten mit einem scheinbar lokalisierten Bronchialkarzinom (pT1–3, pN0, M0)

Summary

Despite an apparently curative resection, about 30–40% of the patients with pT1–3, pNO, MO non-small cell carcinomas (NSCLC) will relapse after surgery. Therefore, it has to be assumed that in some patients a tumor cell dissemination has occurred already at the time of surgery. By using sensitive immunocytochemical techniques, the extent of an early regional and/or systemic tumor cell dissemination can be demonstrated. In pNO patients, an early lymphatic dissemination can be detected in 15.2% of the cases and a systemic spread of tumor cells into the bone marrow in 54.3%. Since systematic mediastinal lymphadenectomy does not significantly improve the long-term prognosis in these patients, a systemic adjuvant therapy should be offered to patients at risk with a stage I NSCLC.

Zusammenfassung

Trotz vermeintlich kurativer Operation erleiden etwa 30–40% der Patienten mit einem pT1–3, pNO, MO-Bronchialkarzinom ein häufig lebensbegrenzendes Tumorrezidiv. Es ist daher anzunehmen, daß bei einem Teil dieser Patienten bereits zum Operationszeitpunkt eine Tumorzelldissemination vorliegt. Mit sensitiven immunzyto-chemischen Methoden kann das Ausmaß einer frühen regionalen und/oder systemischen Tumorzelldissemination nachgewiesen werden. So findet sich bei pNO-Patienten in 15,2% der Fälle bereits eine frühe lymphatische Disseminierung und bei 54,3% eine systemische Disseminierung in das Knochenmark. Da eine systematische Lymphadenektomie in dieser Patientengruppe keine wesentliche Prognoseverbesserung erlaubt, sollten die Risikopatienten im Stadium I einer adjuvanten systemischen Therapie unterzogen werden.
