Von GIGO zu KABA — Grundlagen für eine rationale Infektionsdiagnostik

Summary

In the era of cost containment in health care, medical microbiology also faces the challenge of cost-effective use of microbiological diagnostics. However, problems do not arise primarily within the laboratory during analysis of clinical specimens, but also during the pre-analytical stage, i.e. indication, sampling, storing and transporting of microbiological samples. This situation is emphasized drastically by the acronym GIGO, „Garbage In, Garbage Out“. To overcome this unsatisfactory situation both clinicians and microbiologists have to improve communication and collaboration which is the basis for the rational diagnosis of infection and therapy. Examples of inadequate microbiological investigations together with the correct use of blood cultures are given.

Zusammenfassung

Angesichts der finanziellen Zwänge in der Medizin ist auch die Medizinische Mikrobiologie aufgefordert, vermehrt auf Kosteneffektivität zu achten und die Grundlagen der Infektionsdiagnostik zu hinterfragen und den aktuellen Anforderungen anzupassen. Allerdings liegen die Ursachen für nicht adäquaten Gebrauch der mikrobiologischen Diagnostik nicht nur im Labor, sondern bereits in der präanalytischen Phase, d. h. bei der Indikationsstellung, Probengewinnung, -lagerung und -transport. Der hierbei auftretende Mißstand wird am treffendsten mit dem Akronym GIGO für „Garbage In, Garbage Out“beschrieben. Eine Veränderung dieser Situation ist nur möglich, wenn Kliniker und Mikrobiologen endlich Kommunikation und Kooperation verbessern und damit die Grundlage für eine rationale Infektionsdiagnostik und Therapie schaffen.
