Zusammenfassung

Schmerzen und Beschwerden, die nicht durch eine pathologisch-anatomisch definierbare Läsion verursacht sind, werden im allgemeinen als funktionell bezeichnet. Im Magen-Darmbereich sind funktionelle Beschwerden meist mit Motilitätsstörungen vergesellschaftet, an deren Entstehung neben zentralnervösen, durch das vegetative Nervensystem dem Magen-Darmtrakt zugeleiteten Impulsen, auch die autonomen Plexus, die muskuläre syncytiale Leitung und gastrointestinale Polypeptidhormone beteiligt sind. Zweifellos spielt die aktuelle psychische Konstellation bei der Auslösung und bei der Perzeption funktioneller Magen-Darmbeschwerden eine bedeutende Rolle. Zu den, die Psyche beeinflussenden Momenten gehören nicht selten oligosymptomatische organische Krankheiten, maligne Tumoren, chronische Infekte, hormonale Störungen, die zu einem unbestimmten körperlichen und damit psychischen Mißbehagen führen, das sich dann somatisch als funktionelle Beschwerde fernab vom eigentlichen Krankheitsherd ausdrücken kann. Mit der Feststellung einer funktionellen Störung ist somit nur ein Symptom erfaßt, aber keine Diagnose gestellt. Diese ergibt sich erst aus der Festlegung der seelischen und/oder organischen Ursache, die der funktionellen Störung zugrunde liegt.
