Summary
The detailed analysis of the literature of the last ten years comprising twelve papers concerning pancreatic cancer and ten for esophageal cancer show a clear reduction of postoperative mortality with increasing case volume per year.
Single papers have analysed the main reasons for this phenomenon and showed that postoperative complication rates are lower in high-volume-hospitals and their management of complications is more successful. Further long time prognosis is also correlated to case-volume. Surgery of pancreatic and esophageal cancer is a tast for high-volume-hospitals.
Zusammenfassung
Die detaillierte Literaturanalyse der letzten zehn Jahre von zwölf Arbeiten zum Pankreaskarzinom und zehn zum Oesophaguskarzinom ergibt eine eindeutige Reduktion der postoperativen Mortalität mit steigender Fallzahl pro Jahr. Einzelarbeiten zeigen als Gründe, daß postoperative Komplikationen in den High-Volume-Hospitals geringer und das Management der Komplikationen erfolgreicher ist als in Low-Volume-Hospitals. Weitere Daten weisen darauf hin, daß auch die Langzeitprognose der Patienten aus Kliniken mit hoher Fallzahl besser ist als bei solchen aus Kliniken mit geringer Fallzahl. Tumorchirurgie des Pankreas und Oesophagus ist eine Aufgabe für High-Volume-Hospitals.
