Summary
A new appropach to the treatment of bile duct lesions was investigated. A part of the bile duct was replaced with a venous interponat which had been endoluminally stented by a bioabsorbable stent. A total of 24 pigs was divided into four groups. In group I a segment of the common bile duct was replaced by a venous interponat, while in groups II and III the interponates were additionally stented. Group IV served as control. For investigation of stent degradation, in group II 2 animals were sacrificed after 3, 4 and 5 months, in group III after 6 months. In groups II-IV all animals survived in good condition. The interponat was found to be laminated with bile duct epithel and showed the diameter of the stent, whereas after 4 months the stent material had been completely degraded. The reported technique represents an interesting alternative to the biliodigestive anastomosis because of the preservation of the Sphincter Oddi.
Zusammenfassung
Wir untersuchten eine neue Behandlungsmethode von Defektläsionen des Gallenganges durch Ersatz mit einem Veneninterponat, das mit einem bio-degradablen Stent endoluminal geschient wurde. 24 Schweine wurden auf vier Gruppen aufgeteilt. In Gruppe I wurde ein Gallengangssegment durch ein venöses Interponat ersetzt, in Gruppe I und II zusätzlich gestentet. Gruppe IV diente als Kontrolle. Zur Untersuchung der Stentdegradation erfolgten in Gruppe II Opferungen von jeweils 2 Tieren nach 3, 4 und 5 Monaten, in Gruppe 3 nach 6 Monaten. In den Gruppen II — IV überlebten alle Tiere in gutem Zustand. Das Interponat war mit Gallengangsepithel ausgekleidet und zeigte den Durchmesser des Stents. Nach 4 Monaten war das Stentmaterial abgebaut. Die vorgestellte Technik stellt wegen des Erhalts des Sphincter Oddi eine interessante Alternative zur biliodigestiven Anastomose dar.
