Summary
Aim: In most cases, incarcerated inguinal hernias are treated by conventional inguinal incision, reposition and closure of the hernia defect. We describe a laparoscopically assisted procedure with laparoscopic reposition of the incarcerated mass and conventional hernia repair.
Patients and method: Between October 2001 and September 2002, 224 patients (age: 14 – 93 years) underwent hernia Operation. In 17 patients (7.6%) the Operation had to be done as an emergency due to incarceration, starting in each case with diagnostic laparoscopy. The incarcerated mass could be identified easily and was reposed into the abdominal cavity. An additional resection of the incarcerated mass (small bowel segment, Omentum majus) was necessary in 7 patients. The conventional hernia repair was done without mesh material. Results: Diagnostic laparoscopy helps to identify the incarcerated mass and to decide about a resection. A herniolaparotomy can be avoided in all cases.
Zusammenfassung
Zielsetzung: In der überwiegenden Zahl werden inkarzerierte Leistenhernien konventionell versorgt. Wir beschreiben ein laparoskopisch-assistiertes Verfahren, in dem das Inkarzerat zunächst laparoskopisch identifiziert und reponiert wird. Der Verschluss der Bruchpforte erfolgt konventionell.
Patienten und Methodik: Bei 224 Patienten (Alter 14–93 Jahre), die von Oktober 2001 bis September 2002 wegen einer Leistenhernie operiert wurden, lag bei 17 (7,6%) Patienten eine Inkarzeration vor. Alle Patienten wurden primär laparoskopiert, wobei das Inkarzerat leicht identifiziert und in die Bauchhöhle reponiert werden konnte. Bei 7 Patienten musste zusätzlich eine Resektion des Inkarzerates (Dünndarmsegment, Omentum majus) erfolgen. Die Versorgung der Bruchpforte erfolgte konventionell ohne Netzimplantation. Ergebnisse: Die primäre Laparoskopie ist bei der Beurteilung des Inkarzerates und der Entscheidung über eine Resektion hilfreich, eine Herniolaparotomie kann dadurch immer vermieden werden.
