Zusammenfassung
Bei der Herzinsuffizienz wird die Funktion des Herzens nicht nur von den pathologischen Prozessen des Myokards selbst bestimmt, sondern im wesentlichen auch von komplexen neurohumoralen Regulationssystemen durch Beeinflussung von Vor- und Nachlast. So konnte in den letzten Jahren gezeigt werden, daß vasokonstriktorische neurohumorale Faktoren wie das Renin-Angiotensin-Aldosteron-System, die sympathische Aktivität und das Vasopressin eine wesentliche Rolle spielen, ebenso wie vasodilatatorisch wirksame Faktoren wie das Kininsystem und Prostaglandine. In neuester Zeit galt das wissenschaftliche Interesse besonders einem erst kürzlich entdeckten Hormon, dem atrialen natriuretischen Peptid (ANP), welches aus den Vorhöfen des Herzens freigesetzt wird. In einigen kürzlich erschienenen Reviews wird detailliert auf die Wirkungsweise von ANP eingegangen (3, 7, 8). ANP ist in der Lage, eine durch Vasokonstriktoren induzierte Gefäßkontraktion zu blockieren, außerdem erhöht ANP die Diurese und Natriurese, zum Teil auch die glomeruläre Filtrationsrate und die Filtrationsfraktion. ANP ist außerdem in der Lage, die Renin- und Aldosteronproduktion zu vermindern. Diese Eigenschaften lassen ANP als ein ideales gegenregulatorisches Prinzip zu den oben beschriebenen vasokonstriktorischen Systemen bei der Herzinsuffizienz erscheinen.
