Leistungen der Tumorchirurgie beim Neuroblastom im Kindesalter

Summary

Survival rates in patients with neuroblastoma did not significantly improve during the last two decades. This is particularly true for stage IV tumors. This stage seems to be a tumor fundamentally different from the more localized stages in its biological behavior, and it is not known, whether stage IV tumors originate from a localized stage by metastatic spread. Intensive investigations regarding new therapeutic approaches on an immunological basis or by stimulation of tumor maturation have been unsuccessful by now. Surgery in neuroblastoma has to be adapted to the particular stage to be treated. Certainly, surgery has to be incorporated into a therapeutic strategy which includes the whole spectrum of modern anticancer therapy. Such a therapeutic strategy will be provided by multicenter cooperative studies, such as the neuroblastoma study designed by the German Association of Paediatric Oncology (GPO). A treatment of neuroblastoma without incorporation in a multicenter cooperative study should not be accepted as modern standard.

Zusammenfassung

Trotz intensiver klinischer und theoretischer Forschung ist es bisher nicht gelungen, die Heilungsraten des Neuroblastoms entscheidend zu verbessern. Dies gilt insbesondere für das Tumorstadium IV. Neuere Erkenntnisse deuten daraufhin, daß das Neuroblastom des Stadiums IV ein fundamental anderer Tumor mit anderen biologischen Eigenschaften ist, als das Neuroblastom der lokalisierten Stadien. Versuche, neue Therapieansätze auf immunologischem Gebiet oder durch eine Stimulation der Tumorreifung zu finden, sind bisher fehlgeschlagen. Durch eine Intensivierung der Chemotherapie sind beim Stadium II und III zweifellos Fortschritte erzielt worden. Die chirurgische Behandlung des Neuroblastoms muß sich dem Tumorstadium anpassen: hierauf wird im Detail eingegangen. Sie kann nur integraler Teil eines multimodalen Behandlungskonzeptes sein, wie z. B. die Gesellschaft für Pädiatrische Onkologie (GPO) im Rahmen einer nationalen multizentrischen Neuroblastomstudie ausgearbeitet hat. Eine wie auch immer geartete Therapie des Neuroblastoms ohne Einbindung in eine solche kooperative Studie entspricht heute nicht mehr modernem Standard.
