Mikrochirurgische Übungsmodelle — Organisation und Technik

Summary

The microsurgical practice model is divided into three levels of difficulty. In the plastic model (1) the handling of instruments, various suture materials, anastomotic techniques, and a “microsurgical view” is learnt. In the ex vivo model (2), acquaintance with the anatomical structure and composition of the tissue and its biomechanical properties is provided. In the in vivo model (3) various anastomoses are learnt in structures with a diameter down to 0.4 mm, e.g. vessels, nerves, ureter, and bile duct. Performing an anastomosis should not take longer than 18–20 min. The 14-day patency rate should be 70–100%. Continuous practice and the mastery of microvascular surgery will have a significant impact on the outcome of clinical cases.

Zusammenfassung

Die Übungsmodelle gliedern sich in 3 Schwierigkeitsgrade, die sowohl dem mikrochirurgisch tätigen Kliniker wie auch dem Anfänger gleichermaßen effiziente Aus- und Weiterbildungsmöglichkeiten bieten. I) Übung am Kunststoffmodell: Handling der Instrumente, verschiedener Nahtstärken und Anastomosentechniken, “mikrochirurgisches Sehen”. II) Ex vivo Modell: Kennenlernen von Gewebestruktur und Gewebsaufbau sowie der mechanischen Belastbarkeit. III) In vivo Modell: Verschiedene Anastomosenformen bis zur Grenze des technisch Machbaren (0,4 mm0), Nerven, etc. Anzustreben sind Op-Zeiten von 18–22 min für Einzelknopfnähte/Anastomose. Die 14-Tage-Patency sollte für art. EzE-Anastomosen 100%, für andere Anastomosen 70–80% betragen. Vom sicheren Umgang mit kleinsten Gefäßen durch kontinuierliches Training profitiert die Klinik.
