Passagere Dünndarmverlagerung vor Radiotherapie nach abdomino perinealer Exstirpation

Summary

In cases of low rectal carcinoma, a typical abdominoperineal exstirpation is performed followed by postoperative radiation. An inflatable tissue-expander as used in reconstructive plastic-aesthetic surgery is implanted in the sacral cavity and is covered as far as possible with peritoneum. The expander can be inflated with NaCl solution. The small intestine is kept out of the way by the expander and thus is exposed to minimal radiation. At the end of the radiation series, the expander is deflated and removed through a small incision.

Zusammenfassung

Bei tiefsitzendem Rektumkarzinom wird die typische abdominosacrale Rektumexstirpation mit postoperativer Radiotherapie durchgeführt. Als Ausfüllung der sacralen Höhle wird ein Tissue-Expander, wie er in der rekonstruktiven Mamma-Chirurgie üblich ist, implantiert, und, soweit vorhanden, mit Peritoneum zur freien Bauchhöhle hin abgedeckt. Der Tissue-Expander läßt sich in der Folgezeit durch Auffüllen mit Kochsalzlösung vergrößern, so daß ein Hochverlagern der Dünndarmschlingen bis über das Promontorium, also auch aus dem Bestrahlungsfeld hinaus möglich ist. Nach Abschluß der Bestrahlungsserie wird der Expander abgelassen und durch eine kleine Inzision entfernt.
