Cholelithiasis — interdisziplinäres Therapiekonzept

Summary

An interdisciplinary therapeutic approach in cholelithiasis was presented which is based on 10-year analysis of surgical patients (n = 2038) and experience in interdisciplinary therapeutic approaches to gallstone disease. Both cholecystectomy and cholecystectomy combined with bile duct exploration are standards in elective therapy. There is no alternative to cholecystectomy in cases of acute cholecystitis. Surgical reoperations have been widely replaced by endoscopic methods (except in instances of stenosis and chronic fistula of the bile duct). Duct stone complications and high-risk patients demand endoscopic therapy for choledocholithiasis, if necessary combined with extracorporeal shock-wave lithotripsy (ESWL). ESWL may be an alternative therapy in 15–20% of all patients with cholecystolithiasis.

Zusammenfassung

Die Chirurgische 10-Jahresanalyse (n = 2038) und die interdisziplinäre Zusammenarbeit mit den Kollegen der Med. Klinik II (Endoskopie, extrakorporale Stoßwellenlithotripsie (ESWL) und Lyse) veranlassen uns ein interdisziplinäres Therapiekonzept der Cholelithiasis vorzustellen. Cholecystektomie und Cholecystektomie mit Choledochusrevision sind die elektive Standardtherapie. Die Cholecystitis darf nur chirurgisch behandelt werden. Bei Reeingriffen haben endoskopische Verfahren die chirurgischen z. T. abgelöst (Ausnahme: Gangstenose, chron. Gallefistel). Bei der Choledocholithia-sis zwingen steinbedingte Komplikationen und Risikopatienten zu endoskopischer Therapie ggf. mit ESWL. Die ESWL stellt bei 15–20% der Patienten mit Chlocystolithiasis eine alternative Therapie dar.
