Gastrektomie — Gestern und Heute

Summary

The results of total gastrectomy are characterized by the risk of operation, survival rate as well as the side effects resulting from the loss of the stomach. From 1969–1988, 308 total gastrectomies were performed. In the past 10 years, the death rate was reduced from 24.4% (n = 86) to the current rate of 2.2% (n = 222). The 5-year-survival rate has risen by 10%, up to today’s 26%. Avoiding reflux and constructing a new reservoir must be the most important aims of reconstruction. Preservation of the duodenal passage has metabolic benefits. The esophagojejunostomy (Schloffer) should not be applied, not even in palliative interventions.

Zusammenfassung

Operationsrisiko, Überlebensrate sowie die Nebenwirkungen, die sich aus dem Verlust des Magens ergeben, prägen die Ergebnisse der Gastrektomie. Von 1969–1988 wurden insgesamt 308 Gastrektomien wegen eines Malignom des Magens durchgeführt. Die Letalität konnte in den letzten 10 Jahren von früher (n = 86) 24.4% auf heute (n = 222) 2.2% gesenkt werden. Die 5-Jahres-Überlebensrate stieg um etwa 10% auf heute 26% an. Wichtigstes Ziel der Rekonstruktion muß die Vermeidung von Reflux und die Konstruktion eines neuen Reservoirs sein. Die Erhaltung der Duodenal-Passage hat metabolische Vorteile. Die Oesophago-Jejunostomie (Schloffer) ist auch bei palliativen Eingriffen abzulehnen.
