Die externe patello-tibiale Transfixation bei Patellafrakturen und Abrissen des Ligamentum patellae

Summary

Bony and ligamentous lesions of the distal part of the extensor mechanism require surgical reconstruction. To ensure uneventful healing a long leg cast must be applied in most cases, leading to joint stiffness, muscle atrophy and cartilage damage. To enable early joint motion and partial weight-bearing of the affected leg, the external patellotibial transfixation technique was introduced. Within 3 years (January 1985-December 1987), seven patients with complicated fractures of the patella (compound fractures, avulsions of the lower pole, ruptures of the ligamentum patellae and a post-traumatic osteomyelitis) were treated with this device. Early functional motion started 2 days after the operation. Loading of the osteosynthesis could be enhanded gradually by loosening the clamps. In critical cases when normally long-term immobilization is needed, the treatment has proved very valuable for the joint.

Zusammenfassung

Im Zeitraum vom 1.1.85 bis zum 31.12. 87 wurden in unserer Klinik 7 Patellaosteo-synthesen (Patellartrümmerfrakturen, Polabrisse, Rupturen des Lig. patellae sowie eine Osteitis) durch eine externe patello-tibiale Transfixation versorgt. Die frühfunktionelle Nachbehandlung mit ihren erheblichen Vorteilen für die Ernährung und Trophik aller gelenkbildenden Weichteilstrukturen war bereits 2 Tage nach der operativen Versorgung möglich. Durch das Lösen der Fixationsbacken konnte die Belastung der Osteosynthese sukzessive gesteigert, nach 6 Wochen die Implantate ambulant entfernt werden. In kritischen Fällen, die sonst eine mindestens 6wöchige Ruhigstellung mit destruktiven Folgen für das Gelenk erfordert hätte, hat sich das Verfahren sehr bewährt, da die für die Weichteil- und knöcherne Heilung notwendige Entlastung des Streckapparates problemlos erhalten werden konnte.
