Posttraumatisches Impingementsyndrom nach schultergelenknahen knöchernen Verletzungen

Summary

The impingement (Ip) syndrome of the shoulder corresponds to a diminution of the subacromial space. The main causes of Ip syndrome in juxta-articular osseous injuries of the shoulder are dislocated avulsions of the tubercules, advanced post-traumatic arthrosis of the acromio-clavicalar joint, dislocated fractures of the acromion, injuries of the coracoid process, fractures of the scapula neck and dislocated ventral fragments of the humeral head. An acromioplasty was required in 15 of 22 patients who were treated for Ip syndrome in juxta-articular osseous injuries of the shoulder from 1985 to 1987. Since an Ip syndrome can also be a consequence of juxta-articular osseous injuries of the shoulder, therapeutic success can only be guaranteed if both osseous and soft tissue problems are treated.

Zusammenfassung

Das Engpaßsyndrom am Schultergelenk entspricht einer Einengung des subacro-mialen Raumes. Im Vordergrund der Engpaßsyndrome nach schultergelenknahen knöchernen Verletzungen stehen dislozierte Tuberculumabrisse, fortgeschrittene, posttraumatische Acromioclavicularge-lenkarthrose, dislozierte Acromionfrakturen oder Acromionpseudarthrosen, Verletzungen des Pro-zessus coracoideus, Scapulahalsfrakturen und dislozierte ventrale Humeruskopffragmente. Bei 22 Patienten, die wir von 1985–1987 mit Engpaßsyndrom bei schultergelenknahen knöchernen Verletzungen behandelten, mußten wir bei 15 eine Engpaßerweiterung durchführen. Da ein Impingmentsyndrom auch Folge schultergelenknaher knöcherner Verletzungen sein kann, läßt sich nur durch die kombinierte Behandlung der Knochen- und Weichteilproblematik ein Therapieerfolg garantieren.
