Neue Aspekte in der chirurgischen Therapie des Boerhaave Syndroms

Summary

Boerhaave’s syndrome spontaneous esophageal rupture, occurs almost invariably in the left postlateral wall of the distal esophagus. The initial symptoms are similar to those of myocardial infarction, pancreatitis, pulmonary embolism or perforated ulcer. Twenty patients were treated for spontaneous rupture. All underwent left-sided thoracotomy; 12 patients required only surgical closure of the rupture; 8 patients also needed a laparotomy with distal esophageal resection. The surgical suture was always covered with a fibrin-soaked polyglycol net. Hospital mortality reached 30%.

Zusammenfassung

Die ‘Boerhaave-Syndrom’ genannte spontane Oesophagus-Ruptur betrifft in der Regel die linksseitige posto-laterale Oesophaguswand. Die Klinik ist zu Beginn vergleichbar der eines Myocardinfarktes, einer Pankreatitis oder eines perf. Ulcus. Zwanzig Spontanrupturen wurden von 1974–1987 behandelt. Alle Patienten wurden linksseitig thoracotomiert; bei 12 Pat. reichte eine Über-nähung, bei 8 Pat. war die dist. Oesophagusteilresektion notwendig. Die Naht wurde mit Fibrin getränktem Polyglycol-Netz bedeckt. Die Letalität betrug 30%.
