Die endoskopisch kontrollierte Gastrostomie (PEG) bei mechanischen und funktionellen Schluckstörungen

Summary

PEG is an effective means of providing alimentation to patients who are unable to swallow and for gastrointestinal decompression in patients with chronic postduodenal obstruction. The procedure (FRENTA^® PEG-SET) was performed in 56 patients (oropharyngeal cancer 19 (33.9%), proximal esophageal cancer 7 (12.5%), polytrauma 12 (21.4%), neurologic disorders 8 (14.3%) and postduodenal obstruction 10 (17.9%) with one bleeding complication following needle insertion without sufficient translumination and minor local infections in 8.9%. In cases of obstruction the PEG catheter was replaced 7 days later by an 18 French balloon catheter, pulled through by thread and bougie. Preexistent esophagitis was successfully treated by syringe-administered TAGAGEL^® (4×7 ml +3 ml H₂O/day).

Zusammenfassung

Die PEG ist ein effektives Verfahren zur enteralen Ernährung von Patienten mit Schluckstörungen sowie zur Dekompression bei chronischen therapieresistenten Ileuszuständen. Wir setzten das Verfahren (FRENTA^® PEG) bei 56 Pat. ein (Oropharynx-Ca. 19 (33,9%), prox. Ösophagus-Ca. 7 (12,5%), Polytrauma 12 (21,4%), neurolog. Erkrankung 8 (14,3%) und Dekompression 10 (12,9%). Wir sahen 1 Blutungskomplikation und in 8,9% geringe lokale Infektionen. Zur Dekompression wurde der PEG-Kath. später gegen einen 18 Ch Ballon-Kath. ausgetauscht. Die bei allen Pat. während der PEG diagnostizierte Ösophagitis therapierten wir erfolgreich mit TAGAGEL^® (4×7 ml +3 ml H₂O/Tag.).
