Sonographisch geführte Immunocytologie zur Lokalisationsdiagnostik der Nebenschilddrüsen

Summary

Sixty consecutive patients with hyperparathyroidism (52 pHPT, 8 sHPT) were examined preoperatively using sonographically guided fine needle aspiration and aspirate immunostaining for PTH with a modification of the peroxidase-antiperoxidase technique described by Sternberger. Localisation was successful in 49 patients with pHPT. In two patients adenomas hidden from ultrasound detection were located retrosternally. In three patients with hyperplasia all glands could not be localized due to localisations in the mediastinum. Localisation in the thyroid was clearly detected by immunostaining, as were lipomas, which gave the same ultrasound image as parathyroidadenomas. Sonographically guided fine needle aspiration and aspirate immunostaning are therefore valuable for localizing enlarged parathyroid glands.

Zusammenfassung

60 Pat. mit einem Hyperparathyreoidismus (52 pHPT, 8 sHPT) wurden konsekutiv, präop. einer Lokalisationsdiagnostik mit sonographisch geführter Feinnadelbiopsie mit anschließender Immunocytologie unterzogen. Bei 49 Pat. mit pHPT war die Lokalisationsdiagnose korrekt, bei 2 entzogen sich die E. K. Adenome dem Ultraschallnachweis durch retrosternale Lage. Bei den Hyperplasien konnten bei 3 Pat. nicht alle vergrößerten Drüsen präop. dargestellt werden, da sich einzelne Epithelkörperchen durch ihre Lage der Ultraschallerfassung entzogen. Der intrathyreoidale Lagenachweis und die Unterscheidung zwischen Lipomen, die das gleiche Ultraschallbild liefern, gelingt durch die Immunocytologie. Die sonographisch geführte Feinnadelbiopsie mit PTH-Nachweis ist u.E. die Methode der Wahl zur Lokalisationsdiagnostik.
