Gefäßrekonstruktion bei 1300 Nieren- bzw. Pankreastransplantationen vor der Organimplantation

Summary

From 1976 to 1987, 99 lesions of the vessels were found prior to transplantation of 1309 kidney and 85 pancreas grafts. In 48 cases a direct suture, in 9 cases a segment resection and in 7 cases a free vessel transplantation were performed. Especially lesions of the intima should be either excluded by careful inspection of the donor organs or repaired. In cases of multiorgan explantation, the vessels of the pancreas transplant were elongated with arterial or venous vessels of the donor if necessary. Due to the great shortage of donor organs, an arterial and venous segment should be routinely harvested for potential reconstruction of graft vessels form an allogenic transplant.

Zusammenfassung

Von 1976 bis 1987 wurden bei 1309 Nieren- und Pankreastransplantationen präoperativ 99 Gefäßverletzungen entdeckt. In 48 Fällen wurde der Defekt durch direkte Naht, in 9 Fällen durch Segmentresektion und in 7 Fällen durch eine Freie Gefäß-Tx behoben. Insbesondere Intimaläsionen müssen durch subtile Inspektion der Spenderorgane ausgeschlossen oder repariert werden. Bei derPankreas-Tx wurde nach Multiorganentnahme, wenn erforderlich, eine art. oder ven. Gefäßinterposition durchgeführt. Wegen des großen Organmangels sollte routinemäßig bei der Spenderoperation ein art. und ven. Gefäßsegment entnommen werden, um ggf. ein allogenes Gefäßsegment zur Rekonstruktion zur Verfügung zu haben.
