Spontanpneumothorax und spontaner Hämatopneumothorax

Summary

In a 15-year period 65 patients with spontaneous pneumothorax and three patients with spontaneous hemopneumothorax were treated. Treatment with intercostal drainage and continuous suction was successful in 70% of patients with spontaneous pneumothorax. A thoracotomy was necessary in 11 patients due to incomplete expansion of the lung or persistent bronchopleural fistula. The overall death rate was 3%. Early thoracotomy is mandatory in patients with spontaneous hemopneumothorax and massive bleeding from torn apical adhesions, since bleeding rarely ceases spontaneously.

Zusammenfassung

In 15 Jahren wurden 65 Patienten mit einem spontanen Pneumothorax und 3 Patienten mit einem spontanen Hämatopneumothorax behandelt. Bei mehr als 2/3 der Patienten mit einem spontanen Pneumothorax war die Behandlung mit einer Thoraxsaugdrainage ausreichend. Eine primär operative Therapie wurde wegen inkompletter Ausdehnung der Lunge oder einer persistierenden bronchopleuralen Fistel bei 11 Patienten notwendig. Gesamtletalität: 3%. Beim spontanen Hämatopneumothorax mit massiver Blutung aus zerrissenen apikalen Verwachsungssträngen ist eine frühzeitige Thorakotomie notwendig, da mit einer spontanen Blutstillung kaum zu rechnen ist.
