Konservative oder operative Behandlung des Spontanpneumothorax

Summary

Experience and results in the treatment of 244 cases of spontaneous pneumothorax are reported. The following therapeutic concept was evolved: in all patients a small calibre pleural catheter is first inserted, followed by pleurography and — if possible — a CT scan. If there are massive changes, such as large groups of emphysematous blebs, surgery is the primary mode of treatment. In more localized lesions an attempt at thoracoscopic pleurodesis is made. If this fails, a secondary thoracotomy is advised. In cases without pathological findings chest tube drainage is the only form of treatment.

Zusammenfassung

Aufgrund der Behandlungsergebnisse von 244 Pneumothoraces wird das folgende Therapiekonzept entwickelt: Bei allen Patienten wird nach primärer Einlage eines Pleurocat eine Pleurographie und — wenn möglich — Computertomographie durchgeführt. Finden sich dabei massive Veränderungen, etwa große Blasenkonvolute, dann wird primär operiert. Bei lokalisierten Prozessen versuchen wir dagegen zunächst eine gezielte thorakoskopische Pleurodese, bei Scheitern wird sekundär operiert. Findet sich bei der Diagnostik kein auffallender pathologischer Befund, wenden wir ausschließlich die Thoraxsaugdrainage an.
