Indikation und Technik der Gelenkpunktion am Knie- und Hüftgelenk

Summary

Puncture is indicated for diagnostic and therapeutic considerations. Traumatology has revealed the following indications: a) knee joint: painful posttraumatic or postoperative hemarthrosis, an effusion of unknown etiology, posttraumatic effusion with stable ligaments, suspected infection; b) hip joint: infection (coxitis, hip arthroplasty) and undefined joint pain. The technical realization presupposes the principles of asepsis. Knee-joint punctures are placed supra-patellarly from lateral or medial, infra-patellarly para- or perligamentous; hip joint punctures are placed laterally or ventrally. The major complication is joint infection.

Zusammenfassung

Die Indikation zur Punktion wird aus diagnostischen und therapeutischen Überlegungen gestellt. In der Traumatologie ergeben sich folgende Indikationen: a) am Kniegelenk: der schmerzhafte posttraumat. bzw. postop. Hämarthros, der Erguß unklarer Ätiologie, der posttraumat. Erguß bei stabilem Bandapparat, der Infektverdacht; b) am Hüftgelenk: der Infekt (Coxitis, TEP) u.d. unklare Hüftgelenkschmerz. — Die technische Durchführung setzt die Beachtung der Grundprinzipien der Asepsis voraus. — Punktionsstellen am Kniegelenk: suprapatellar von lateral oder medial, infrapatellar para- oder perligamenter. Das Hüftgelenk kann von ventral oder lateral punktiert werden. — Die wesentliche Komplikation ist der Gelenkinfekt.
