Die abdominozervikale Ösophagusextirpation mit einzeitiger Gastroplastik

Summary

The method of choice in surgery for cancer of the lower thoracal section of the esophagus and carcinoma of the cardiac esophagus is esophagectomy without thoracotomy followed by isoperistaltic posteromediastinal gastroplasty. If indicated this operative technique can be used for cancer of the middle thoracal section of the esophagus (good tumor regression following radiation) and in corrosive esophageal strictures. Forty-six patients underwent operations: 28 for cancer, 18 for corrosive esophageal strictures. While the method of the operation was modified, its technical principles are as follows: (1) the A. gastrica dextra is bisected at the pylorus; (2) the duodenum is mobilized according to the method of Kocher; (3) an incision perpendicular to the greater curvature of the stomach is made just under 2 cm above the pylorus, and 2.5–3 cm diameter tubes are formed parallel to the greater curvature; (4) a laser-suture apparatus is used at all stages of the operation; (5) the transplantate is affixed to the surrounding tissues and on the paravertebral fasciae at the outlet of the posterior mediastinum so as to ensure that pus does not enter the posterior mediastinum in case the sutures of the anastomosed esophagus open; (6) the throat wound and the mediastinum are drained by two-lumen drainage tubes. Three patients died from pulmonary embolism and abscesses of the mediastinum. The esophageal anastomosis at the cardia failed in three patients but did not cause death.

Zusammenfassung

Die Ösophagektomie ohne Thorakotomie mit einzeitiger isoperistaltischer posteromediastinaler Gastroplastik ist die Methode der Wahl in der chirurgischen Behandlung des Karzinoms im unteren thorakalen Ösophagusabschnitt und des kardioösophagealen Karzinoms. Bei Indikation kann diese Operation auch beim Karzinom des mittleren thorakalen Ösophagusabschnittes (gute Tumorsregression nach der Bestrahlung) und bei den Verätzungsstrikturen angewandt werden. 46 Patienten wurden operiert: 28 mit Karzinomen, 18 mit Verätzungsstrikturen des Ösophagus. Die Methodik der Operation ist modifiziert, ihre technischen Grundsätze sind folgende: 1. die A. gastrica dextra wird am Pylorus durchschnitten; 2. das Duodenum wird nach Kocher mobilisiert; 3. 2cm oberhalb des Pylorus senkrecht zur großen Kurvatur und ohne diese 2 cm zu erreichen, wird der Magen inzidiert und die Röhre mit einem Durchmesser von 2,5–3 cm wird parallel zur großen Kurvatur gebildet; 4. in allen Stufen der Operation wird der Laser-Nähapparat angewandt; 5. am Ausgang aus hinterem Mediastinum wird das Transplantat zirkulär an den Nachbargeweben und an der paravertebralen Fascia fixiert, was der Eitereindringung ins hintere Mediastinum im Falle der Nahtinsuffizienz der Ösophagusanastomose vorbeugt; 6. die Halswunde und das Mediastinum werden mit zweilumigen Drainagen drainiert. 3 Patienten verstarben wegen Lungenembolie und Mediastinumabszeß. Nur bei 3 Patienten entwickelte sich die Insuffizienz der Ösophagusanastomose am Hals (ohne Letalausgänge).
