Summary

Operative methods of closing decubital ulcers, such as direct closure following excision; skin transplantation; cutaneous, muscular and musculocutaneous flaps; as well as free microvascular tissue transplants, are shown and the advantages and disadvantages of these methods discussed. Muscular and musculocutaneous flaps are the best forms of tissue substitute. The criteria for choosing an operative method are based on the evaluation of the wound, knowledge of the etiology, prognosis, as well as social background of the patient.

Zusammenfassung

Die Operationsverfahren zur Deckung von Decubitalulcera, nämlich der Direktverschluß nach Exzision, Hauttransplantation, cutane, muskuläre und muskulokutane Lappenplastiken sowie die freie mikrovaskuläre Gewebetransplantation werden dargestellt und deren Vor- und Nachteile diskutiert. Muskel und Muskelhautlappen bieten den am stärksten belastbaren Gewebeersatz. Kriterien für die Auswahl des geeigneten Operationsverfahrens sind neben der Beurteilung der lokalen Situation die genaue Kenntnis der Ätiologie, der Prognose, sowie des sozialen Umfeldes des Patienten.
