Chirurgische Behandlung maligner Weichgewebstumoren des Retroperitoneums

Summary

From 1982 to 1986 52 patients were treated for retroperitoneal soft tissue sarcomas. Since the tumors caused only vague symptoms, 73% of the patients already had a palpable abdominal mass. Contrast-enhanced CT was the most useful diagnostic aid. Seven patients underwent only explorative celiotomy. Palliative resection was performed in 30, curative resection in 15. Multiorgan operations were mandatory in 39 patients. Sixteen histological varieties of sarcomas were encountered. The death rate of inpatients was 1.9%. A followup was done in 38 adult patients: the median survival time after exploratory laparotomy was 3 months, after palliative resections 32 months. All patients with curative resection were still alive after a median observation time of 22 months.

Zusammenfassung

Von 1982–86 wurden 52 Pat. operiert. Bei uncharakteristischen Symptomen kamen 73% d. Pat. mit bereits tastbarem Tumor zur Therapie. Als beste diagnostische Hilfe erwies sich die kontrastmittelverstärkte CT. Bei 7 Pat. war nur eine Probelaparotomie möglich; 30 Pat. wurden palliativ, 15 kurativ operiert. In 39 Fällen waren komplexe Multiorganoperationen erforderlich. 16 verschiedene histologische Diagnosen wurden gestellt. 1 Pat. verstarb postoperativ: Letalität 1.9%. Die 38 erwachsenen Patienten wurden nachbeobachtet: die mediane Überlebenszeit nach PL betrug 3 Monate, nach Palliativeingriffen 32 Monate. Alle 11 kurativ operierten Patienten waren nach median 22 Monaten noch am Leben.
