Palliative Therapie des Pankreas-Karzinoms — eine interdisziplinäre Aufgabe

Summary

Pancreas carcinoma has the worst prognosis of all carcinomas of the GI tract, having a five-year-survival rate of 0.4%. The primary methods of treatment are palliative. The resection rate in our own number of patients totaled 17% from 1960 to 1987; in 52% of the cases, bypass procedures were carried out. Alternatively, endoscopic (n = 131) or transcopic (n = 80) drainage of the bile duct can be performed. The aim of all these procedures should be the alleviation of pain according to the WHO scheme or a CT-controlled neurolysis of the celiac plexus.

Zusammenfassung

Das Pankreas-Karzinom hat mit einer 5-Jahresüberlebensquote von 0,4% die schlechteste Prognose aller gastroint. Karzinome. Pall. Behandlungsmethoden stehen oft im Vordergrund. Die Resektionsquote im eig. Krankengut betrug zwischen 1960–87 17%; in 52% wurden Bypass-Verfahren durchgeführt, die mittl. Überlebenszeit war 9,8 bzw. 4,3 Monate. Alternativ wurden endosk. (n = 131) oder transk. (n = 80) Gallengangsdrainagen durchgeführt. Allen Verfahren gemeinsam sollte eine gezielte Schmerzbekämpfung nach dem Stufenschema der WHO bzw. eine CT-gesteuerte Neurolyse des Plexus coeliacus (n = 16) sein.
