Entwicklung eines Rotationskatheters zur dynamischen Angioplastie

Summary

We developed three milling catheters (2.3, 3.0 and 3.5 mm Ø) consisting of a rotating head that is connected to a high-speed driving motor (frequency up to 80,000 rpm) via a flexible cable. The rinse fluid from numerous holes within the head has cooling as well as antithrombotic properties. The catheters were employed percutaneously (2.3 mm) and in conjunction with vascular surgery to treat occlusions of the superficial femoral and the popliteal artery when impassable to a guide wire. The dynamic recanalisation was successful in eight patients with severe ischemia. Two patients underwent a femoropopliteal bypass. The special construction of this device reduces the risk of perforation or emoblisation.

Zusammenfassung

Wir entwickelten 3 Fräskatheter (2,3 3,0 u. 3,5 mm 0), die aus einem rotierenden Kopf bestehen, der mit einem regelbaren Elektromotor (Frequenz bis 80 000/min) über ein flexibles Kabel verbunden ist. Die Spülflüssigkeit tritt aus zahlreichen Öffnungen an der Spitze aus und hat zugleich kühlende als auch antithrombotische Eigenschaften. Die Katheter wurden perkutan (2,3 mm) und in Verbindung mit einem gefäßchirurgischen Eingriff benutzt zur Behandlung von Verschlüssen der A. femoralis superficialis, die mit dem Führungsdraht nicht passierbar waren. Bei 8 Patienten war die Rekanalisation erfolgreich; 2 Patienten erhielten einen femoro-poplitealen Bypass. Dieses spezielle System ist so konstruiert, daß eine minimale Perforations- und Embolisationsgefahr besteht.
