Summary

The clinical behaviour of malignancies is characterized by four stages of progressive development: local, locoregional, mono- or oligotop-distant, and disseminated-distant. In the first three stages surgery is the most effective means of treatment. The possibility of tumor cell dissemination and implantation within the surgical area determines the methods of oncological surgery (no touch, wide margins of clearance, en bloc resection together with lymph node dissection).

Zusammenfassung

Die Tumorkrankheit ist durch einen Ablauf in vier Phasen gekennzeichnet: 1. lokalisiert, 2. lokoregionär, 3. mono- oder oligotope Fernmetastasierung in Leber bzw. Lunge, 4. disseminierte Fernmetastasierung. Phase 1–3 sind Domäne der Chirurgie, die das lokoregionär am sichersten wirksame Verfahren darstellt. Die Möglichkeit der Tumorzelldissemination im Operationsgebiet hat wesentliche Konsequenzen für die onkologische Operationsmethodik (no touch, weite Sicherheitsabstände, Resektion und Lymphknotendissektion en bloc).
