Die Wertigkeit der Computertomographie, Endoskopie und Sonographie im praeoperativen Staging des Oesophagus-und Cardiakarzinoms

Summary

Forty-four patients cancer of the esophagus and cardia were preoperatively staged by CT, endoscopy and ultrasonography. Results were compared with intraoperative findings. CT had a high sensitivity, detecting lymph node metastases in compartments II and III (70–80%) and tumor infiltrating the diaphragm, bronchial system and lung (80–100%). The sensitivity of ultrasonography was low for perigastric lymph node metastasis (0–57%). Endoscopy revealed correct tumor length in only 21%. There was no evidence of unresectability even in stage IV. Ten percent of the patients were staged as false positive. Explorative laparotomy or thoracotomy therefore remains an important procedure in treatment of cancer of the esophagus and cardia.

Zusammenfassung

44 Patienten mit Oesophagus- und Cardiakarzinom wurden mit CT, Endoskopie und Sonographie untersucht, die Ergebnisse mit dem intraoperativen Befund verglichen. Das CT erfaßte Metastasen und Organinfiltrationen mit einer Sensitivität von 70–80% bzw. 80–100%. Die Sensitivität der Sonographie war gering (0–57%). Die Tumorlängenbestimmung durch die Endoskopie war in 21% richtig. Der Nachweis der Irresektabilität war nicht sicher möglich. 10% wurden falsch positiv befundet. Die explorative Laparotomie oder Thorakotomie bleibt wesentlicher Bestandteil der Behandlung des Oesophagus- und Cardiakarzinoms.
