Resektion von Hilustumoren

Summary

Local resectability of hilar tumors can usually only be assessed intraoperatively after dissecting the bile duct and its bifurcation as well as the hepatic artery and portal vein branches. The extent of resection depends on the exact tumor localisation, particularly the biliary tributaries of 1st and 2nd order, and eventual vascular invasion or infiltration of the parenchyma. Hilar resection can be performed alone or in combination with partial left or right hepatectomy. Subsequent reconstruction by hepaticojejunostomy uses transhepatic stents.

Zusammenfassung

Die lokale Resektabilität von Hilustumoren läßt sich oft erst intraoperativ beurteilen, wobei der Gallengang bis zum Leberhilus und die Äste von Leberarterie und Pfortader dargestellt werden. Das Ausmaß der Resektion richtet sich nach der Tumorlokalisation und Ausdehnung in Gallengangsäste 1. und 2. Ordnung und einer eventuellen Gefäß- oder Parenchyminfiltration. In Betracht kommt entweder die alleinige Hepaticusgabelresektion oder Kombination mit Leberteilresektion links oder rechts. Die Rekonstruktion erfolgt als Hepaticojejunostomie mit Einlage transhepatischer Drainagen.
