Angiographie-orientierte Leberresektion

Summary

Main trunks of the portal vein are situated in the dorsal quarter of the liver and the main branches in the transition to the dorsal third. The operation is planned on the basis of angiographic X-rays. Arterial vascular anomalies must be heeded. Embolic tumour thrombosis should be suspected if branches of the portal vein beyond the tumour are occluded. Since the tumours are located in the area of the hepatic veins and adjacent to the vena cava, pre-operative digital subtraction angiography of these veins is recommended. Resection of the liver is especially dangerous in this area. Selective coeliacography and mesenteriography taking both paths of portal vessels into consideration determine the course of the operation. The interpretation of the X-rays may be difficult because they give only a two-dimensional view.

Zusammenfassung

Gefäßstämme verlaufen im dorsalen Viertel der Leber, die Hauptgefäßstämme im Übergang zum dorsalen Drittel. Nach den Angiogrammen wird der Operationsablauf vorgeplant. Arterielle Gefäßanomalien sind zu berücksichtigen. Verschlossene Pfortaderäste außerhalb der Tumorstrukturen sind verdächtig auf Tumorthromben. Bei Lage der Tumoren im Lebervenenbereich und der V. cava anliegend ist die praeoperative Lebervenen- und V. cava-Darstellung anzuraten, da gerade in diesen Bereichen die wesentlichsten Gefahren der Leberresektion bestehen. Grundsätzliche selektive Coeliacographie und Mesentericographie unter Miterfassung beider Portalgefäßstraßen sichert den dann planbaren Operationsablauf. Deutungsschwierigkeiten bei zweidimensionaler Darstellung sind möglich.
