Summary
Standard techniques of hepatic resections include all procedures that follow the segmental and lobular anatomy of the liver. A transabdominal approach is sufficient to execute any type of resection. Initial exploration includes search for extension of the hepatic lesion, extrahepatic involvement and identification of the vascular supply. Any anatomical resection starts with a hilar dissection, followed by ligation of the appropriate hepatic artery, portal vein branch and hepatic duct. Before transparenchymal dissection is persued, the appropriate hepatic vein is isolated, completing the isolation of the anatomical lobe. Following the line of demarcation, the parenchyma is dissected with any technique familiar to the surgeon, i.e. finger fraction technique, Cousa-employment, laser technique, blunt scissor technique or electrocautery technique. Pringle-maneuver can be executed safely for 30 min. Hemorrhaging from the resected surface can be controlled by infrared coagulation, application of tissue glue, mattrice sutures of omentoplasty. Extended hepatectomies, isolated segmentectomies, vascular reconstructions or ex-vivo benching is not within the scope of standard hepatic resections.
Zusammenfassung
Standardtechniken der Leberresektion umfassen alle, der segmentalen und lobären Anatomie der Leber folgenden Eingriffe. Der Zugang erfolgt ausschließlich über eine abdominelle Inzision (Bilateral-subkostal). Nach palpatorischer Orientierung beginnt die Dissektion mit einer Hilus präparation und Unterbindung der entsprechenden Lebergefäße (Arterie Pfortader und Gallengang). Vor der Parenchymdissektion erfolgt die Isolation und Übernähung der abführenden Lebervene. Entlang der Demarkationsgrenze wird das Parenchym in einer dem Chirurgen vertrauten Methode durchtrennt. Blutungen aus der Resektionsfläche werden durch gezielte Umstechungen, Infrarotlichtkoagulation der Oberflächenversiegelung mit Gewebekleber versorgt. Erweiterte Hepatektomien (rechts- oder linksseitige), isolierte Segmentektomien, längere Eingriffe mit Pringle-Maneuver, Gefäßrekonstruktionen oder Ex-situ Präparationen gehören derzeit nicht zum Standardvorgehen sondern in die Hände erfahrener Spezialisten.
