Summary
Two patients of 142 suffering from Crohn’s disease (CD) and peritonitis (of 122 patients) developed perforations of the small bowel during acute episodes of CD (→ drainage). During open dorsoventral abdominal lavage both patients lost albumin (→ 6–8 FFP/day) due to CD. One patient died from respiratory failure (18th day). Open abdominal lavage of the other was stopped, and the open abdomen was sprinkled with water; a loss of albumin could no longer be observed. The patient survived (34 days with artificial ventilation, 49 days ICU).
Zusammenfassung
Bei 2 Patienten (Pat.) mit Morbus Crohn (MC) und Peritonitis (2 von 142 MC-Pat. bzw. von 122 Peritonitis-Pat. der letzten 5 Jahre) entwickelten sich im akuten Schub Dünndarmfisteln (→Drainage). Im akuten Schub traten hohe Eiweißverluste (→6–8 FFP/Tag) bei offener dorsoventraler Bauchspülung auf. Ein Pat. verstarb im Lungenversagen (18. Tag). Bei dem anderen Pat. wurde die offene Bauchspülung nach 6 Tagen abgebrochen und der offene Bauch durch Berieselung feucht gehalten. Ein Eiweißverlust fand danach nicht mehr statt, der Pat. überlebte (34 Tage Beatmung).
