Die Langzeit pH-Metrie bei Patienten mit Ulzera duodeni: Prä- und postoperative Analyse nach proximal gastraler Vagotomie

Summary

Thirty-five patients with duodenal ulcer (DU) and five with DU and pyloric ulcer (mean age 42 years) were subjected to intragastric 24-h pH measurement pre- and postoperatively. Determinations of pH in the antrum were recorded every 9 sec for 24 h. The operative procedures consisted of proximal gastric vagotomy in DU patients; an additional antrectomy was performed in patients with pyloric ulcers. The preoperative pH pattern was very heterogeneous: in some patients pH in the antrum was very low, while others did not show “ulcerogenic” acidity. In the postoperative state a similar heterogeneous pattern was observed, with the exception of patients with a pyloroplasty who had permanent pH values above 5. Intragastral 24 h pH measurement may be useful for postoperative monitoring after proximal gastric vagotomy.

Zusammenfassung

35 Pat. mit Ulkus duodeni (UD) und 5 Pat. mit UD und Ulkus ad pylorum mit einem Durchschnittsalter von 42 J. wurden prä- und postoperativ einer intragastralen 24 h pH Metrie unterworfen. Die pH-Werte wurden im Antrum alle 9 sec. bestimmt. Die Operationsverfahren bestanden in proximal gastraler Vagotomie bei den UD Pat., die Pat. mit Ulkus ad pyloreum erhielten zusätzlich eine Antrectomie. Die pH Profile waren präoperativ sehr inhomogen mit teils sehr niedrigen pH Werten im Antrum, während andere keine „ulzerogene“ Azidität im Antrum aufwiesen. Das gleiche galt für die postoperativen pH Profile mit der Ausnahme der Patienten, die eine Antrectomie erhalten hatten. Hier fanden sich langanhaltende pH Werte über pH 5. Die intragastrale Langzeit pH Metrie kann eine nützliche Methode zur postoperativen Kontrolle nach proximal gastraler Vagotomie sein.
