Diagnostik der postoperativen Dünndarmfunktion mittels des H2-Atemtestes

Summary

The intestinal transit time was determined in 12 patients following proximal gastric vagotomy and in 24 patients after gastrectomy before, 1, 3, 5 and 7 days after the operation. A significantly prolonged small bowel transit time was observed up to three days after vagotomy and up to seven days after gastrectomy. During this time there was abnormal bacterial overgrowth in the small bowel in 37.5% of the patients with gastrectomy. Therefore, depending on the severity of the operation an intestinal dysfunction can last for up to seven days after surgery.

Zusammenfassung

Die intestinale Transitzeit wurde mittels H₂-Atemtest präoperativ und am 1., 3., 5. und 7. postop. Tag bei 12 Patienten nach proximal gastraler Vagotomie und bei 24 Patienten nach Gastrektomie bestimmt. Eine signifikant verlängerte Transitzeit zeigte sich nach Vagotomie bis zum 3. und nach Gastrektomie bis zum 7. postop. Tag. Bei letzteren ließ sich in 37,5% der Fälle in diesem Zeitraum eine bakterielle Fehlbesiedlung des Dünndarmes nachweisen. In Abhängigkeit von der Schwere des operativen Eingriffes wird demnach eine bis zum 7. postop. Tag dauernde Dünndarmfunktionsstörung verursacht.
