Einfluß verbesserter Diagnostik auf Prognose des C-Zell-Karzinoms der Schilddrüse

Summary

Medullary thyroid carcinoma (MTC) secretes polypeptides such as calcitonin (CT), CGRP and catacalcin. From 1978 to 1987, 52 patients were operated on for MTC. Nine patients with occult tumors showed preoperatively elevated CT levels. In eight of these patients CT levels were normalised after adequate surgical treatment. Only 3 of 42 patients with clinical signs of the disease had postoperatively normal CT levels. The defection of tumor markers in patients with MTC in the occult tumor stage permits curative surgical treatment.

Zusammenfassung

Eine herausragende Eigenschaft des C-Zell-Karzinoms der Schilddrüse ist die Synthese und Sekretion von Polypeptiden wie Calcitonin (CT), CGRP und Katacalcin. Von 1978–1987 haben 52 Pat. mit einem C-Zell-Karzinom operiert. Bei 9 Pat. lag bei präoperativ erhöhten CT-Werten ein okkulter Tumor vor. 8 von diesen Pat. wiesen nach adäquater chir. Therapie normale CT-Werte auf. Von 42 Pat. mit klinisch manifesten Tumoren hatten nur 3 p.op. normale CT-Werte. Der Nachweis von Tumormarker bereits im okkulten C-Zell-Karzinomstadium ermöglicht eine kurative chirurgische Therapie.
