Das septische Abdomen. Primäre Behandlungsverfahren

Summary

Therapy for septic abdominal wounds comprises pre-, intra- and postoperative measures. Preoperative acute measures include infusion and antibiotic and analgesic therapy together with monitoring of all vital organ functions. Operative therapy is directed at irradicating the focus of infection. This includes intraoperative lavage, debridement, bowel decompression and drainage. Postoperative measures are intensive-care monitoring and continuation of the initiated infusion and antibiotic therapy. Continuous peritoneal lavage and all forms of programmed lavage can be applied in individual cases in which operative therapy seems to be of doubtful success.

Zusammenfassung

Die Therapieverfahren beim septischen Abdomen umfassen prä-, intra- und postoperative Maßnahmen. Zu den präoperativen Akutmaßnahmen gehören die Infusions-, Antibiotika-und analgetische Therapie mit Kontrolle sämtlicher vitaler Organfunktionen. Die operativen Maßnahmen schließen die Herdsanierung und adjuvante Operationsmaßnahmen ein. Dazu gehört die intraoperative Lavage, das Debridement, die Darmdekompression und die Drainage. Zu den postoperativen Therapieverfahren zählen die intensivmedizinische Überwachung mit Fortsetzung der eingeleiteten Infusions- und Antibiotikatherapie. Die kontinuierliche Peritoneallavage und alle Formen der programmierten Lavage sind Therapieverfahren, die bei operativ nicht sicher sanierbaren Peritonitiden individuell eingesetzt werden können.
